Quest requisition forms are not valid at any other lab facility, and attempting to use a Quest requisition order at an alternative facility may result in a costly bill that you are responsible for.
If you require a Labcorp requisition form due to infeasible geographic constraints or preference, please verify your zipcode and email info@marekhealth.com with the tests you'd like to order. We may be able to manually create this order and requisition form for you. Do not place an order on Marek Diagnostics as this will create a Quest requisition form. Please note that there are some pricing differences using labcorp tests. We can provide a quote upon inquiry.
