Once you receive your lab requisition form, you can schedule an appointment at your nearest Quest Diagnostics location to have your labwork drawn. You can find and book your nearest location using Quest's location finder — just enter your zip code to see nearby centers, hours, and appointment availability.

If you require a Labcorp requisition form due to infeasible geographic constraints or preference, please verify your zipcode and email info@marekhealth.com with the tests you'd like to order. Quest requisitions are not valid at Labcorp. We may be able to manually create this order and requisition form for you. However, please note that there are some pricing differences using labcorp tests. We can provide a quote upon inquiry.
